Abstract

¸ñÀû: º» ¿¬±¸ÀÇ ¸ñÀûÀº ¿Ü°úÁßȯÀÚ½Ç È¯ÀÚÀÇ ¼¶¸Á ¾ÆÇüÀ» ÆľÇÇÏ°í, ¼¶¸Á ¾ÆÇü¿¡ µû¸¥ ´ë»óÀÚÀÇ Æ¯¼º°ú °ü·Ã ¿äÀÎÀ» ÆľÇÇϱâ À§ÇÔÀÌ´Ù.

¹æ¹ý: ÀÚ·á¼öÁýÀº ÀÏ ´ëÇк´¿ø ¿Ü°úÁßȯÀڽǿ¡¼­ 2018³â 10¿ùºÎÅÍ 2019³â 6¿ù±îÁö ½ÃÇàÇÏ¿´´Ù. ¼¶¸ÁÀº CAM-ICU·Î, ¼¶¸Á ¾ÆÇüÀº DMSS-4·Î ÃøÁ¤ÇÏ¿´´Ù. ´ë»óÀÚ Æ¯¼ºÀº ÀüÀÚÀǹ«±â·ÏÀ» ÅëÇØ ¼öÁýÇÏ¿´°í, µ¥ÀÌÅÍ ºÐ¼®Àº ±â¼úÅë°è¸¦ »ç¿ëÇÏ¿´´Ù.

°á°ú: ¿¬±¸±â°£ ÀÔ½ÇÇÑ ÃÑ 1,112¸í °¡¿îµ¥ ¼¶¸Á ȯÀÚ´Â 172¸íÀ̾ú´Ù(15.5%). Ä¡¸Å ȯÀÚ¿Í ¿¬±¸ Âü¿©¸¦ °ÅºÎÇÑ È¯ÀÚ¸¦ Á¦¿ÜÇÏ°í, 126¸íÀÇ ´ë»óÀÚ°¡ ÃÖÁ¾ ºÐ¼®¿¡ Æ÷ÇԵǾú´Ù. ¼¶¸Á ¾ÆÇüÀº °úÈ°µ¿¼º 32.5%, ÀúÈ°µ¿¼º 42.9%, È¥ÇÕ¼º 11.9%, ¾ÆÇü ¾øÀ½ ¼¶¸ÁÀÌ 12.7%À̾ú´Ù.

°á·Ð: º» ¿¬±¸ °á°ú ¿Ü°úÁßȯÀڽǿ¡¼­ ÀúÈ°µ¿¼º ¼¶¸ÁÀÌ °¡Àå ¸¹¾Ò°í, ÀΰøÈ£Èí±â Àû¿ë, ÁøÁ¤Á¦ Åõ¿©, Ä«Å×ÅÍ º¸À¯ °³¼öµµ ÀúÈ°µ¿¼º ¼¶¸Á ȯÀÚ°¡ °úÈ°µ¿¼º ¼¶¸Á ȯÀÚº¸´Ù ¸¹¾ÒÀ¸¸ç °£È£ÁßÁõµµµµ ´õ ³ô¾Ò´Ù. ±×·¯¹Ç·Î, ±¸Á¶È­µÈ µµ±¸¸¦ »ç¿ëÇÏ¿© Á¶±â¿¡ ¼¶¸Á ¾ÆÇüÀ» »çÁ¤ÇÏ°í, °¢ ¾ÆÇü¿¡ ÀûÇÕÇÑ °£È£ ÁßÀ縦 °³¹ßÇÏ´Â °ÍÀÌ ÇÊ¿äÇÏ´Ù.

Purpose: The purpose of this study was to investigate the motor subtypes of delirium in patients in a Surgical Intensive Care Unit (SICU), and identify the factors related to the characteristics of patients according to the motor subtypes of delirium.

Methods: Data were collected in the SICU of a tertiary hospital in * city from October 2018 to June 2019. Delirium was detected using the Confusion Assessment Method for the ICU (CAM-ICU) and motor subtypes of delirium were measured with the Delirium Motor Subtype Scale (DMSS)-4. Patients¡¯ characteristics were obtained by using the electronic medical records. Descriptive statistics were used to analyze the data.

Results: Among 1,112 patients, 172 patients showed delirium (15.5%). After excluding dementia patients and patients refusing to participate in the study, 126 patients included in the final analysis. Delirium patients were classified as hyperactive delirium (32.5%), hypoactive delirium (42.9%), mixed delirium (11.9%), and non-motor subtype delirium (12.7%).

Conclusion: The study results suggest that hypoactive delirium is the most prevalent motor subtype of delirium in SICU. More application of ventilators, more administration of sedatives, more use of catheters, and higher nursing severity were reported for hypoactive delirium cases than hyperactive ones. Therefore, it is necessary to assess early the motor subtypes of delirium using structured tools and develop appropriate nursing interventions suitable for each subtype of delirium.
